Advanced Childbirth Assistant Skills - Midwife Assistant Training
Three Day Intensive Training Workshop and Certification
Purpose and Benefits of this Course
The Advanced Childbirth Assistant Training is designed to give the doula, labor companion or registered nurse skills and knowledge to assist midwives with prenatal care, births, newborn and postpartum care at home and birth center settings, and to be able to offer Enhanced Labor Support (monitrice support) to women giving birth in hospitals.
Benefits of Basic Doula Support
Several studies report that Doula support reduces rates of cesarean section, reduces labor induction, decreases length of labor, descreases use of pain medication, increases sucess of breastfeeding as well as improving maternal-infant interaction and lowering postpartum depression and increases maternal satisfaction with chilbirth experience (Kennel and Klaus, 1991).
Who Should Attend?
RNs, LVNs, nursing students, medical students, doulas, childbirth educators, student midwives, aspiring midwives, midwife assistants and apprentices, health educators, EMTs, CNAs, HHAs, and any woman who wants a career supporting pregnant and birthing women in homebirth and birth center settings. You do not have to have given birth yourself to attend. Attendance at 5 births prior to the training is recommended. Must have taken a Basic Doula Training prior to this course. Registered Nurses, Licensed Vocational Nurses, Emergency Medical Technicians or Certified Nursing Assistants, and Home Health Aides with no prior experience with prenatal care, labor, delivery or postpartum and newborn care must take a basic doula skills course prior to taking this course.

Recommended Pre-Course Training
- Certification in phlebotomy/venipuncture by a California-approved program. The course should include venipuncture theory and skills practicum. This skill is not required to be a Midwife Assistant, but, if you are enrolled in a midwifery training program or plan to enroll in the future, and are taking this course to obtain an apprenticeship, it is strongly recommended you certify as a phelebotomist.
Pre-Course Requirements
- Basic doula training (completion of a training is required, but certification is not required prior to beggining this course).
- Attendance at a childbirth preparation series (or childbirth educator certification with ALACE, ICEA, Lamaze, Bradley, Birthworks, or other).
- Neonatal Ressucitation Certification (Please contact Sue Turner, LM at Email: stmidwife@aol.com, for dates of upcoming courses in Ventura, CA).
- IV Certification: required only If you are a RN or midwifery student curretly enrolled in a California Medical Board approved midwifery school. (Please contact Sue Turner, LM, at Email: stmidwife@aol.com, for upcoming IV Certifcation courses in Ventura, Ca).
Learner Objectives
By the completion of all course requirements, particpants will:
- Define the roles of the midwife, midwife’s assistant, doula and advanced childbirth assistant/monitrice
- Compare and contrast three features of the midwifery and medical models of maternity care
- Describe three major developments in the history of midwifery and natural childbirth movement in the United States
- Describe the scope of practice for midwifery assistants, apprentice midwives, advanced childbirth assistants/monitrices and doulas
- Define the types of midwives practicing in the U.S. and their educational backgrounds
- Describe three advantages and disadvantages of the places of birth: hospital, birth center and home
- Describe the relationship between obstetricans, nurses, midwives and midwife assistants, doulas and advanced childbirth assistants/monitrices
- Discuss common cultural baises and differences and their impact upon the midwife assistant-client relationship.
- Demonstrate the Three Step Strategy for communication.
- Describe the key features of bereavement counseling clients after an infant loss
- Describe a solution to an ethical problem in midwifery care or doula support
- Describe six complications and emergencies of childbirth and state the appropriate plan of action for out-of-hospital management
- Define medical terminology used in maternity care
- Describe the rationale for and demonstrate skills in blood pressure, pulse, temperature, and fetal postion (lie, presentation, postion)
- Demonstrate how to take fetal hearttones using a doppler, fetoscope and pinard horn
- Demonstrate skills to assist midwives with set up of IV therapy, IM injections and suturing
- Demonstrate how to document fetal heart tones on a chart
- Describe the rationale for specific prenatal testing and procedures
- Identify female reproductive anatomical structures of pregnancy, labor and birth
- Demonstrate assisting skills for postpartum examination of mothers and infants
- Demonstrate charting of information obtained in postpartum examination
- Describe basic breastfeeding management
- Demonstrate techniques for assisting with latching on of infants at the breast
- Discuss strategies for obtaining and keeping a midwifery apprenticeship
- Plan and participate in a midwifery community education event
Post-Course Requirements for Certification
- Pregnancy or Childbirth Research Paper
- Community Service Activity (group project)
- Attendance at six births and completion of three self-evaluations and three client evaluations and one midwife, doctor or nurse evaluation.
Course Specifics
The 24-hour 3-day training meets Friday, Saturday and Sunday from 9:00 am to 6:00 pm (with one hour lunch). The Los Angeles location is TBD. CEUs for RNs and CCE's pending.
Includes training manual, supplies and lunch each day.

Breastfeeding Peer Counselor Training
Three Day Training and Certification
This training is available for individuals or agencies to give their perinatal health workers lactation support skills.

Purpose of the Course
The purpose of this course is to give women who want to support other women in breastfeeding their babies the skills and information to do so and to address barriers to breastfeeding initiation and duration. You do not have to have personal experience with breastfeeding to take this course.
Increasing breastfeeding initation and duration in the United States is one of the Healthy People 2020 objectives for the public's health. Breastfeeding has numerous benefits for mothers and children including lowering rates of infant mortality and morbidity, enhances bonding between mother and infant, and decreases women's risk of breast, cervical and uterine cancer.
Who Should Attend?
Aspiring lactation educators, aspiring lactation consultants, perinatal case managers, Certified Perinatal Health Workers (CPHWs), Community Health Outreach Workers (CHOWs), health educators, doulas, childbirth educators, childbirth assistants, midwife assistants, midwifery apprentices, aspiring midwives, student midwives, student nurses, nurses, medical students, mothers, grandmothers, any woman who wants to help other women breastfeed.

By the end of this training, participants will be able to:
- Discuss common cultural baises and differences and their impact upon the counselor-client relationship.
- Demonstrate the Three Step Strategy for communication.
- Describe three benefits of breastfeeding to infants and mothers.
- Describe the importance of father's support in breastfeeding success and describe two ways to involve fathers in supporting the breastfeeding dyad.
- Describe the barriers that confront breastfeeding mothers.
- Describe the nutritional components of breastmilk and their purpose.
- Describe the phases and types of breastmilk and their purpose.
- Identify the anatomical structures of the lactating breast.
- Describe the role of hormones in the physiology of breastfeeding.
- Describe basic breastfeeding management.
- Demonstrate three ways to position baby at the breast.
- Demonstrate techniques for assisting with latching on of infants.
- Describe four problems encountered by breastfeeding mothers and four remedies for these problems.
- Demonstrate pumping and storage of breastmilk and describe safe milk handling guidelines.
- Describe three special situations that may inhibit breastfeeding and describe three ways to support breastfeeding mothers.
Certification Requirements
- Pass certification exam with score of 70%.
- Assist three mothers with lactation support and provide three self-reports and three client reports.
Specifics of Training
The course meets for 18 hours, 3 days, 9 hours per day (with a one-hour lunch). Trainings are Friday, Saturday and Sunday from 9:00 am to 6:00 pm. Los Angeles location TBD. CEUs for RNs and CCE's pending.
Includes training manual, The Breastfeeding Answer Book, and lunch each day.

About the Instructors

Cordelia Hanna-Cheruiyot, MPH, CHES, CCE, CBA
A Certified Childbirth Educator, Certified Birth Assistant who since 1991 has taught Childbirth Preparation Classes to hundreds of couples in private practice, clinics, public health, hospital and community settings, and attended births as a Primary M idwife Under Supervision, Assistant Midwife and Labor Companion/Monitrice in homes, birth centers and hospitals. Cordelia has trained Doulas and Breastfeeding Peer Counselors since 2002. She obtained her Bachelor of Arts degree in Theatre and Dance from Indiana University in 1986. In 2008, she received her Masters in Public Health (MPH) in Health Education and Promotion/Maternal Child Health from Loma Linda University. She is a Certified Health Education Specialist (CHES) acredited by NCHEC. She is also a wife and mother of two home-born children.
Shelly Girard, LM, CPM, MPH
A California Medical Board Licensed Midwife and Certified Professional Midwife accredited by the North American Registry of Midwives, Shelly has been a homebirth midwife since 1972 and has delivered over 4000 babies. She was one of the first midwives to become licensed when California's licensed midwifery law was enacted in 1994. She has trained several apprentices and assistants in her Los Angeles homebirth service, Childbirth at Home A Labor of Love. She received her Bachelor of Science in Biology and Psychology from Tufts University in 1972 and earned a Masters in Public Health (MPH) from UCLA in 2000. She is a mother of two home-born sons.
